How to order your home medication:

Your doctor will have given you either an NHS prescription or alternatively a private prescription. We can dispense both types of prescription but you need to ensure you complete the prescriptions correctly.

Complete your prescription as detailed below:

NHS prescription: If you are exempt from paying prescription charges, you’ll need to tick the appropriate box on the back of the prescription form and sign and date it where indicated. Sometimes you may need to supply additional Proof of Exemption. Where this is necessary, please send a signed photocopy of the document indicated in the Proof of Exemption table (adjacent to the code letter you’ve ticked on the back of your prescription), rather than the original.

If you are not exempt from paying prescription charges, please sign the prescription in the appropriate place. You will then need to pay the relevant prescription charge to us; this is currently £6.85 per item (see Payment Details below ).

Private prescription: Please complete the Information Request Form and post it together with your prescription and payment to Central Homecare Limited.

Place the completed Information Request Form, your prescription and a cheque (where applicable), into an envelope and post it to Central Homecare Limited, Unit 6 Grove Park, Mill Lane, Alton, Hampshire GU34 2QG. By sending a prescription to Central Homecare Limited, you consent to the pharmacy dispensing your order. If you wish to know who will dispense your medication, please contact Central Homecare Limited on 01420-543400 and you will be provided with this information.

Payment details:

If you intend to pay by credit or debit card, please complete the bottom section of the Information Request Form. Alternatively, please make your cheque payable to Central Homecare Limited

Delivery details:

FREE DELIVERY for all UK prescriptions. In general, we will despatch your prescription in plain and discreet packaging, through the Royal Mail’s Recorded Delivery or Special Delivery service (depending upon the value of the drugs). For drugs which need to be kept cold, your delivery will be made by a specialist courier company (please note that these cold chain items will be supplied in specialist packaging so that they are kept at the correct temperature). In all cases you will need to sign to confirm receipt of your prescription. When you receive your drugs, please ensure you store them according to the directions on the packaging.
